Print the form linked below or pick up a form from the Registrar's Office in Student Services in A-Building. Requests should be emailed to sue.sorrentino@yourcharlotteschools.net or returned to the Registrar's Office. Call our registrar at 941-575-5450, ext. *7006 for assistance.
Transcript Request for Former Students
If your college is a Florida Public College, please encourage them to use FASTER if you graduated 2010 and after.
If graduation or attendance occurred in 2010 or later, you may pick up a form from the Registrar's Office in Student Services in A-Building or print the form linked below, complete it, scan and email to sue.sorrentino@yourcharlotteschools.net
